How its used:

It has been specially designed to encourage documenting known significant clinical findings prior to a client being in dates for birth and creating ease for clinicians at the time of a transfer of care. This form can then be shared physically with EMS and receiving hospitals.
Receiving Hospitals:
Hospitals may also use this form for initial triage phone intakes.
