Perinatal Transitions Community Birth Practice Registration
Home birth and birth center practices across Maine are invited to join a Perinatal Transitions.
The Perinatal Transitions Program works to enhance client safety and satisfaction during the transition from planned out-of-hospital births to hospital by improving communication and coordination during transfers of care. By developing shared protocols, collecting and analyzing transfer data, and strengthening collaboration among out-of-hospital providers, EMS, and hospital teams, we aim to make every transfer smoother and safer for families and providers alike.
Across the country, programs like this are helping create more consistent and compassionate transfer experiences. Forming local committees by facility ensures individualized and tailored community needs are supported.
Each committee includes:
Hospital facility clinical liaison
Local EMS representative
Regional out-of-hospital community-based perinatal provider
Committees meet annually to review data from the Perinatal Transitions Program, share insights, and refine tools and practices that improve perinatal care transitions.
